GLL is looking for Health Referral Facilitator to work across the sport and leisure facilities, and schools in Hillingdon on our Child and Family Weight Management service. As this is a new service, we are looking for a number of positions (full time, part time and casual) and are looking for instructors that specialise in physical activity, nutrition and/or both. This job advert is specifically for a part time position. Salary shown is for a full time position and will be pro-rated to part time hours. 

 

The Health Referral Facilitator role is responsible for delivering our Xplore programme (5-13years), healthy lifestyle workshops in schools, and a physical activity programme specifically for teenagers.

Our programmes are aimed at children, young people and their families which combine nutritional workshops with group-based physical activity.

 

The Health Referral Facilitator is responsible to the Health Referral Coordinator – Children and Young People.

 

What you’ll do:

  • To ensure the smooth day-to-day running of the programmes, maximising effectiveness of all available resources.

  • Carry out health and lifestyle assessments with referral children and young people and develop safe and appropriate programmes of physical activity / exercise.

  • Act as first point of contact for children and young people (and their parents) referred onto the scheme

  • To provide individuals with evidence-based, relevant and culturally sensitive nutritional information regarding a healthy lifestyle.

  • To constructively interact with different people in a variety of areas within the operational team, wider health teams and GLL support services.

What you need:

  • GP Referral qualification (Level 3) or equivalent nutrition or physical activity coach qualification

  • A recognised Fitness instructor or Personal training qualification (Level 3)

  • Flexibility to work afternoons and evenings (3-7pm)

  • Experience of working with children, young people and their parents in groups

  • A good understanding of healthy lifestyles in children and young people

  • Ability to motivate individuals and support behaviour change

  • Good time management and punctuality

  • Ability to travel across the borough
