GLL is looking for a Health Referral Administrator to work from the Waterfront Leisure Centre in Woolwich. Based within the administration  ‘hub’ for the Healthwise Healthy Lifestyle Service, the administrator will be responsible for the smooth and effective administration for the Healthwise and service providing a telephone support service, high levels of customer care  as well as accurate of data management. The administrator will also act as a support service to the staff delivering the Health interventions which includes a Physical Activity Referral Scheme, an Adult Weight Management Scheme and a Children and Young Peoples Healthy Lifestyle Service.

 

What you need:

  • Ability to communicate clearly and to a variety of individuals including participants, leisure Centre staff and referring professionals.

  • Have an awareness of the needs of and an ability to communicate with participants.

  • High levels of accuracy in relation to data management and inputting.

  • High levels of IT competency.

  • Ability to motivate individuals and support behaviour change.

  • Ability to work as part of a team and/or on own initiative.

  • Good time management and punctuality.

  • To work a shift pattern in line with the needs of the service (currently this is outlined as being scheduled to be 8am to 8pm Monday – Friday but may change with service development)

What you'll do:

  • Receiving, screening and distributing referral for approval.

  • Contacting all referrals  to confirm receipt of referral.

  • To flag to the Healthwise Coordinator/Manager/Clinical practitioner any referrals that may not be appropriate for onward referrals.

  • Providing a telephone support function for those referred to the Healthwise service

  • Responding to participant queries, escalating any issues where necessary, through a telephone support service as well as face to face queries.

  • Taking messages from the answer phone and responding accordingly

  • To book appointments, where appropriate for group and individual sessions with the Healthwise team.

  • Timely access to assessment (waiting time from referral to assessment) ensuring this meets the contractual target.

  • Act as a point of contact for referrals to key partners.

  • To send reminders about appointments (telephoning participants where appropriate).

  • Provide feedback and information to referrers and participants on the programme including other local health pathways.

  • Following up participants in relation to activity levels following an intervention and recording this information on the database.

Desirable but not essential:

  • A good knowledge of the health and fitness industry

  • A good understanding of the barriers to exercise and effective strategies to support behaviour change

  • A good understanding of physical activity interventions .

  • Experience of working within the Leisure/Fitness industry

  • Experience of working in a telephone call centre environment dealing with customer enquiries
